Request for access to mine plans: Application form

This form to be completed by the current mine owner, duly appointed site senior executive or exploration manager, or any other person if they provide written permission for access to mine plans from the current tenement owner, site senior executive or exploration manager.

Note: All other applicants are to submit a freedom of information application.
