Significant Incident Summary No. 4 Driver entrapment risk when exiting a bus or coach – fatal incident

WorkSafe has conducted an investigation into an incident where a bus driver was entrapped between the door and frame of the bus, causing fatal injuries. There have been several reported incidents across Australia of workers being fatally entrapped in bus doors.
The bus had a single pneumatic door on the left-hand side, which could be operated by switches located under the front bumper, on the driver’s instrument panel or through an emergency release switch on the footwell between the driver’s seat and the door.

Health and Safety Bulletin No. 15 Hydraulic fluid contamination resulting in uncontrolled movement: elevating work platforms

WorkSafe is aware of recent incidents where hydraulic fluid contamination has resulted in the uncontrolled movement of the basket on elevating work platforms (EWPs). This includes the loss of performance related to platform level and jib function.

Hydraulic fluid contamination can occur from the use of incorrect oil, dirt entering the hydraulic system or metallic fragments in the hydraulic system due to wear or damage.

First steps to farm safety: Guide

Farms are unique workplaces. The combination of hazards and the nature of the work makes agriculture one of the most dangerous sectors in which to work. These transitional and saving measures apply from the commencement of the Work Health and Safety Act 2020 (WHS Act) and Work Health and Safety (Mines) Regulations 2022 (WHS Mines Regulations). Measures that transition or preserve existing arrangements in the Mines Safety and Inspection Regulations 1995 (the MSI Regulations) or delay commencement of certain duties in the WHS Mines Regulations can be found in regulation 2 (Commencement) or Part 11.4 Transitional and savings provisions (transitional).

Mines statutory positions register

The Mines statutory positions register is used to validate an individual’s statutory position examination or certificate under the Work Health and Safety (Mines) Regulations 2022. Statutory positions are appointed by the person conducting a business or undertaking (PCBU) to carry out specific functions in relation to the operations of a mine.

The online register allows users to search legislation exam results, statutory certificate holders, or search holders of certificate of competency.
