Information submitted through this form is handled securely and treated as confidential.
LGIRS will not share your contact details with external parties without your consent. However, there are circumstances where information you provide may need to be discussed with the workplace
Please be aware that:
- If an investigation proceeds, a WorkSafe inspector may contact the workplace or the person conducting a business or undertaking (PCBU) to request information.
- As a result, the workplace may become aware that concerns have been raised, even if your identity is not disclosed.
- LGIRS takes steps to protect your identity wherever possible, but complete anonymity cannot be guaranteed.
In particular:
- The workplace may infer who raised the concern based on existing knowledge or circumstances.
- To ensure procedural fairness, the workplace must be informed of the matters raised so it can respond appropriately. Although WorkSafe does not release your name, aspects of the report may allow the workplace to infer your identity.
- Workplace documents such as rosters, records or witness statements may indirectly identify individuals involved.
In some cases, enforcement decisions may be subject to external review, including by the Work Health and Safety Tribunal, where informants or witnesses may be asked to provide evidence.
WorkSafe may use de-identified information from reports for regulatory analysis, reporting and continuous improvement.
Freedom of information
Information held by WorkSafe, including Inspector notes, emails and other records, may be accessible through a formal application under the Freedom of Information Act 1992 (WA).
