Background Details:
The defendant was an owner-builder of a double storey dwelling. An investigation on 25 March 1997 found that there was inadequate edge protection and lack of coverage of penetrations in the concrete floor slabs. Formwork timber had nails protruding and scaffolding had midrails and toe-boards missing.
